PHYLLIS ANN (nee Miller), Dec. 8, 2015. Wife of Gary. Mother of Amy Forman (Jeffrey Fischer) and Benjamin (Lorine) Forman. Sister of Steve Miller (Olga Boyko) and Ruth Miller. Grandmother of Harrison Fischer, Carter and Max Forman. Mrs. Forman was a devoted Preschool Teacher in the Philadelphia School System for over 25 years, most recently at Fitzpatrick Elementary School. Relatives and friends are invited to Funeral Services Friday, 1 P.M. precisely GOLDSTEINS' ROSENBERG'S RAPHAEL SACKS SUBURBAN NORTH, 310 Second St. Pike, Southampton. Int. Shalom Memorial Park. Shiva will be observed at the residence of Benjamin and Lorine Forman. In lieu of flowers, contributions in her memory may be made to Fox Chase Cancer Center, 333 Cottman Ave., Phila., PA 19111.
